the IICF office with the following supporting documents:<\/p><ul><li>National Identity card \/ Birth certificate of the victim<\/li><li>National Identity card \/ Birth certificate of the claimant (if not the victim).<\/li><li>Proof of address for the victim (MCB\/SBM Statement or Utility Bill)<\/li><li><span style=\"font-size: 16px;\">Copy of witness details &amp; statement if any<\/span><\/li><li><span style=\"font-size: 16px;\">Copy of police report if available<\/span><\/li><li><span style=\"font-size: 16px;\">Death certificate (if applicable)<\/span><\/li><li><span style=\"font-size: 16px;\">Affidavit of filiation \/ deed of notoriety (if
