How to Submit a claim

The victim/claimant needs to fill a Hit and Run claim form available online or at the IICF office with the following supporting documents:

• National Identity card / Birth certificate of the victim
• National Identity card / Birth certificate of the claimant (if not the victim).
• Proof of address for the victim (MCB/SBM Statement or Utility Bill)
• Copy of witness details & statement if any
• Copy of police report if available
• Death certificate (if applicable)
• Affidavit of filiation / deed of notoriety (if applicable).
